PC-PTSD-5 · 2-minute screening

Free Online PTSD Test (PC-PTSD-5)

This free PTSD test uses the PC-PTSD-5, a brief 5-question screen developed for the U.S. Department of Veterans Affairs to identify people who may have post-traumatic stress disorder. It takes about a minute and is completely confidential.

If you have experienced a frightening, traumatic, or life-threatening event, answer based on how you have felt in the past month. A positive screen does not mean you have PTSD — but it does mean a full evaluation is worthwhile. Heartland treats PTSD by secure video across Texas.

This screening runs entirely in your browser. Your answers are private — they are not saved, sent, or shared.

PTSD Test

First, one question

Sometimes things happen to people that are unusually or especially frightening, horrible, or traumatic. For example: a serious accident or fire; a physical or sexual assault or abuse; an earthquake or flood; a war; seeing someone killed or seriously injured; or having a loved one die through homicide or suicide. Have you ever experienced this kind of event?

Does a positive screen mean I have PTSD?

No. The PC-PTSD-5 is a screening tool, not a diagnosis. A positive result means it is worth talking with a licensed provider, who can complete a full evaluation and recommend the right care.

Can PTSD be treated through telehealth?

Yes. Evidence-based PTSD care — including therapy approaches and, when appropriate, medication management — can be delivered effectively by secure video. Heartland serves patients throughout Texas.

Is my information private?
